Child Care Safety Tools Overview

The Office of Child Care provides a variety of tools to find additional information regarding child care in Oregon. In the menu above and links below, you will find the following resources:

CHILD CARE SAFETY PORTAL

CHILD CARE SAFETY PORTAL OVERVIEW

The Child Care Safety Portal is a resource for parents and families to check on the safety and quality of licensed child care programs in Oregon. The Portal lets you search for a child care provider and view their licensing history over a period of time.

The Portal is meant to add to, but not replace, other sources of information. For more information on finding and choosing child care, please go to our Find Child Care page.

To receive a customized referral to child care programs contact 211info by:

  • Calling 211. Listen to the prompt for Child Care and press the specified number.
  • Texting the keyword “children” or “niños” to 898211 (TXT211).
  • Emailing children@211info.org.

From the referral list, you can choose the Child Care program that best suits your needs.

WHAT YOU WILL SEE ON THE CHILD CARE SAFETY PORTAL

  • Child care license status, license type, and license capacity
  • Summary of inspection visits including:
    • Valid findings within the last ten years
    • Unable to substantiate findings within the last two years
  • Complaints – Disclaimer: A complaint does not imply the allegation(s) is/are valid.
  • Enforcement activity
  • Number of serious injuries or deaths (For details Click Here to open the Injury Log)
  • Quality program participant status (known as “Spark”)

DEFINITIONS

  • Finding: The Office of Child Care’s written conclusion based on assessments of a potential or observed violation.
  • Valid Finding: There is evidence that the violation occurred.
  • Unable to Substantiate Finding: There is conflicting evidence, or evidence is not available, on whether the violation occurred.  The Office will issue a finding that it is Unable to Substantiate a complaint when there is conflicting evidence of equal weight or sufficient evidence is not available to determine whether the violation occurred. A complaint does not imply the allegation(s) is/are valid.
  • Serious Injuries: When an injury occurred that needed attention from a licensed health professional, such as a physician, EMT or nurse. Licensed child care providers are required to report these injuries to the Office of Child Care within five days of the occurrence.
  • Spark Rating: Spark is a statewide resource used to help improve the quality of child care in Oregon. Spark ratings are shown for programs with a rating of 3, 4, or 5. For more information visit SparkOregon.org.

ADDITIONAL INFORMATION

  • Not all child care providers must be licensed by the Office of Child Care.
  • Every licensed facility receives at least one announced inspection and one unannounced inspection every year.
  • Child care providers have the right to request a review of findings as noted in the Office of Child Care Findings Review Procedures.

UPDATES TO THE CHILD CARE SAFETY PORTAL ARE MADE EVERY TUESDAY MORNING BY 8 A.M. PST.

Frequent visitors to the Child Care Safety Portal should regularly clear their internet/mobile browser’s cache to be sure that the information displayed is up to date.

HOW TO MAKE A COMPLAINT

CHILD CARE HOMES AND CENTERS

An individual wishing to register a complaint against a provider or child care center may submit the on-line complaint form (above) or call the Office of Child Care’s Central Office at 1-800-556-6616.

You will speak to a compliance specialist who will take your information and follow up on your complaint. You may also contact your Licensing Specialist who will pass the complaint on to our compliance specialist for intake.

An individual inquiring about the compliance history of a home-based child care can call the Office of Child Care’s Central Office at the 503-947-1400 or 1-800-556-6616.

An individual inquiring about the compliance history of a center-based child care can call the facility’s Licensing Specialist directly or the Office of Child Care’s Central Office to obtain the name and phone number for the facility’s Licensing Specialist. The name of the Licensing Specialist for the facility would be displayed on the facility information screen.

EMERGENCY SUSPENSIONS

The Office of Child Care has suspended the following licenses. For additional information, including a list of compliance history with injury reports, legal actions and more, please contact the Office of Child Care at 1-800-556-6616.

If a license was previously listed on the Emergency Suspension list and no longer appears it may indicate:

  1. The Emergency Suspension action was withdrawn and is no longer in place; or
  2. The license has expired and the issue that led to the Emergency Suspension has not been resolved by the Office of Child Care.

Please contact the office of child care at 1-800-556-6616 for any questions.

Emergency Order of Suspension are redacted in accordance with ORS 181.534, 192.435, 192.437, 409.255, 419B.035(7) and OAR 414-061-0040(2) and will be posted when available.

  1. The Emergency Condition(s) action was withdrawn and is no longer in place; or

View Order Imposing Emergency Conditions

INJURY LOG

Serious Injury or Incident

Facilities must report a serious injury or incident within 5 days after it occurs. As of October 15, 2018 serious injury or incident is defined in OAR 414-180-0010, OAR 414-205-0010, OAR 414-300-0005, and OAR 414-350-0010 as any of the following:

• Injury requiring surgery
• Injury requiring admission to a hospital
• Injury requiring emergency medical attention
• Choking and unexpected breathing problems
• Unconsciousness
• Concussion
• Poisoning
• Medication overdose
• Broken bone
• Severe head or neck injury
• Chemical contact in eyes, mouth, skin, inhalation or ingestion
• All burns
• Allergic reaction requiring administration of Epi-Pen
• Severe bleeding or stitches
• Shock or confused state
• Near drowning

Exceptions include the following: Injuries for which a child is evaluated by a professional as a precaution; Injuries for which first aid is administered at the facility, but no further treatment by a medical professional is warranted; or Medical events due to routine, ongoing medical issues, such as asthma or seizures.

Injury Log

This injury log includes injuries that occurred at licensed child care facilities where attention from a licensed health professional, such as a physician, EMT, or nurse was sought. Facilities are required to report these injuries to the Office of Child Care within seven days of the occurrence.
